-The back of the insert has an image taken from an earlier version of the game:

On the insert, this screenshot is missing the text above the life bars for all characters, including both player's scores.

-Terry's rising tackle -or "rising upper cut" as the manual calls it -is incorrectly listed as being up, down, A. It should be down, up, A. The Japanese version's manual also has this error.

-Similarly Andy's sho ryu dan, or "flying guillotine", has the same mistake -it's listed as up, down, A when it should be down, up, A.
