Different titles and fonts. The Japanese version is a remake of the original game. However, when the game was released overseas, Konami of America decided to change the story so it was a sequel to Simon's Quest, and changed the title of it appropriately.

In the intro sequence, their is a small pool of blood on the floor in the Japanese version. This was removed from the American version.

Another thing to look out for in the intro are the graves. All of the crosses were removed from the American version and part of the background was changed to a gate. "Dracura" was also removed.
