When you arrive in Little Italy, all you can do is run forward -the streets are blocked off by debris, but the path to the church is clear. When Kyle gets to it, he finds Sims waiting near the entrance. Sims asks Kyle where his sister is. Kyle tells him that she's somewhere safe. Sims reveals that he only has between 20 and 30 men left, and nobody has any idea what is waiting for them inside that church.

Sims sends in part of his SWAT team, but as three guys get to the church entrance, Bella appears and drops a grenade! Kyle yells for them to pull back, but his warning comes too late. The grenade goes off, killing some of the SWAT team and blasting Kyle and Sims off their feet.

When the cutscene ends, a load of enemies will run out of the church, and the only way to progress is to defeat them all. Sims will help you, and he'll shoot at enemies with his pistol. Three SWAT team members will also provide cover, but unlike Sims, they can (and will) die. Some of the enemies will carry grenades, so stay well away from them before they explode. Whilst there are many enemies in this section, it isn't that hard due to the abundance of weapons you can find. Once everyone has been beaten, head inside the church.

As Kyle and Sims head inside, they come across Bella, who is reciting from the Book of Revelation. He also appears to have created four horsemen. Chang walks in, telling Bella that this is insanity. Bella tells her that he is the instrument of God's will. He walks up to her and draws out a sword, telling her not to say such things, or she will be punished for her sins.

Chang resumes working on the horsemen, injecting them with more glow. Bella tells his sons to rise up, as the hour of revelation is at hand. His four horsemen of the apocalypse have come to smite this wicked world, as the Bible foretold. Kyle then steps forward, demanding to know where his brother is.

Bella tells him that his brother is gone -gone with the foul stench of human frailty. Kyle can now either repent... or die! Just then War awakens, and Kyle realizes that War is Nicky "Weasel" Wissell! War grabs Kyle by the face and throws him through a window.

War also heads outside, and then the fight against him begins. War will be in the middle of the arena, but you cannot hurt him with your fighting techniques -this is a gun only battle. There are guns scattered around the floor, but be careful picking them up. War is armed with a gatling gun, which is also capable of shooting grenades. When you hear the gun's barrel start to spin, take cover behind one of the larger gravestones. You can also hide behind trees, but War's bullets can destroy them. Don't try to run around when War is firing, as you'll get hit.

As soon as War stops firing, quickly move in and start your own attack. Try picking up a grenade or molotov cocktail and using those first before using guns, as you'll do more damage this way. Always make sure to lock onto war before firing, so you don't waste any ammo. The weapons do eventually respawn so don't worry about running out of them. When you defeat War, his head and arms will explode, destroying him.

Kyle will head back inside the church. You will be in a short corridor, run through it and collect the first aid kit and energy bar. As you approach another door, a cutscene will start. Kyle cautiously walks up to the door and opens it slightly. Inside the room is Bella's Famine -who used to be Blades! Famine is slicing up the bodies of Sims' men.

And then Famine drags over Sims himself! Sims sees Kyle and tells him to run, but he is then killed by Famine. Kyle runs into the room, but is too late. Famine then sees him and the next battle begins. Famine is extremely fast, and uses knives to fight with. You need to land some quick combos in, such as the strong attack, light attack, light attack combo, as these will hit Famine enough times to make him stumble back away from you.

This can be difficult due to Famine's speed. He has an attack where he will stretch out his arms to try and grab you, and if he manages this, he'll drag you in to headbutt you. He will also try throwing his knives at you if you are a distance away from him. There are lots of weapons scattered about the floor, but I suggest leaving them until the boss is low on health, so you can use them to finish him off quick.

Famine has two other attacks to watch out for. If he suddenly leaps up into the air, stay away from him as he'll pound the ground, causing you to fall over if you are too close. Also, if you are close to him, he will sometimes use a move where he spins around a few times and moves very quickly towards you. Keep hitting him with quick combos and make use of the weapons (especially the grenades). Famine will eventually collapse and die.

Kyle walks back into the church, but only finds Chang and a huge monster. Chang says she could have accomplish such great things, but she's been left with nothing but the ravings of a drug-crazed priest. Working with Bella may have been an error of judgment on her part, but she still has her most cherished conception. Kyle says he'll give her one more chance to help him, or he'll come up there. Chang tells him that he won't be able to do that, and then sets her creation, Pestilence, on him!

Pestilence is a giant monster with multiple arms. Whenever he walks he'll leak some acid behind him, so don't step into that. Your moves don't do much damage to him, so stay back from him. Pestilence can throw bones at you, and will also sometimes vomit acid everywhere around him, so again, stay back. What you need to do is wait until he starts releasing several green projectiles. These will explode after a few seconds, but you can pick them up and throw them back at him.

These do large amounts of damage to Pestilence and you will most likely be able to throw a couple whenever he releases any. Keep this up and Pestilence will soon fall. But it isn't over yet, as Pestilence will shed part of his skin and get back up. Pestilence is now a lot faster, and has a new set of attacks. He can throw several small projectiles at you, one after the other, when you get close. His other projectile attack is to gather several of them and throw them all at you at once.

Another attack Pestilence has is to pick up one of the benches dotted around the floor and try and smash you with it. When he does this, an item will often appear in the wreckage, such as health items or weapons. There are two ways to attack Pestilence: try to lure him into using his combo (which consists of a knee, a swipe and a punch) by getting close to him, then backing off. After he does his punch, move in and land some hits. The other way to attack is to wait until you can pick up a weapon, and start shooting him with it.

This can be a long fight, especially if Pestilence starts spamming his projectiles. Keep at him and eventually his body will rip itself apart. Pestilence will melt away into nothing.

On the church's roof, Bella is still going on about how the hour of judgment is upon them, and woe be to him who does not carry the sign of the Lord! He says that people should not see him as a savior, but as a dark angel of death, sent to cast all into the pits of hell. Kyle arrives and tells Bella he's the only one going to hell. Bella sees that Cane has come looking for his Abel. He feels Kyle's pain, as he also had a brother, one he loved as deeply as Kyle's. He had come to this brother seeking a better life, but it destroyed him. Kyle says he doesn't give a damn about Bella's family, but Bella gives a damn about his. Cody is the one who killed his brother!

The circle is complete, and now Bella will avenge his fallen brother. His own suffering has led to inspiration, and that inspiration has led to... Death! Cody appears, now completely changed by glow! Kyle tells Bella he is a sick bastard, and he'll pray for his own death when he's through with him. Bella says that his name isn't Bella. His real name is Belger!

The fight against Cody Death begins. Cody has several powerful attacks. At long range he will use a power wave, which creates a line of fire on the ground for a few seconds. At close range, Cody can punch, summon a small tornado or try to grab you.

To attack Cody, get close to him and then back up. He'll try and attack you, so as he makes his move, run around him and then start your own attack. Use a combo or a move like the drop kick to make Cody stumble back away from you. Then repeat this process a few times. Cody isn't actually that difficult to beat, but if you are struggling, you can get Cody to help you out. Lure him near the boxes around the floor and get his attacks to hit them -they'll break apart, often revealing items. After Cody falls, Kyle tries to get his brother to remember who he is, but Belger appears and says that Death will continue this battle at his side.

You now have to fight Cody and Belger. However, you need to deal with Cody first. He's gained a new attack where he sends down several projectiles -if you see him raise his arms into the air, stay away from him to avoid being hit. He'll also throw out two power waves at the same time rather than just one. You need to deplete Cody's life bar so he collapses, so use the same tactics as before. But watch out for Belger -he'll throw grenades into the arena, and occasionally use a bazooka. You can get Cody to walk around near the grenades when they go off, as they will do big damage to him. When Cody's life bar is emptied, he'll collapse.

At which point Belger will enter the fight. You need to do as much damage to him as you can, whilst trying to keep him away from Cody. Bella only uses weapons in battle, including an SMG. He cannot block, but whenever you knock him down, stay back -when he gets up he'll swipe at you with his sword.

If Belger pulls out his SMG, try to take cover behind the cross to avoid being hit. If Belger manages to get to Cody, he'll inject him with more glow to revive him. When this happens, Belger will back off and cannot be hit, so you'll need to beat down Cody again. Repeat this process until Belger has been defeated.
