In 1992, a second SNES port of Final Fight was released in Japan. This version was later released in America as a Blockbuster rental exclusive, and is called Final Fight Guy. Final Fight Guy adds Guy as a playable character, but at the cost of removing Cody. Any other shortcomings with the original port are still present here, which means the Industrial Area is missing, there's no 2 player mode, and you can only get up to 3 enemies on the screen at once.

As Cody is missing and Guy is present, the intro was altered. It now has Guy state that Cody is still in Japan, explaining why he's absent. The ending was also changed, and has Jessica talk to Guy during it. Enemy placement was altered, the cheat menu is slightly different and two new items have also been added -one that grants temporary invulnerability, another that gives you an extra life. Another small change is that a layering error in the original version was corrected -in the bar of West Side, the original version had a group of people missing. This was fixed for Final Fight Guy, and the crowd appear at the bar.

The American version of this game is quite valuable, a boxed copy with instructions is now worth hundreds of dollars at auction.
