Toki thinks about how he has left the savior role to Kenshiro. As he does this, lots of radioactive ash enters the shelter and Toki realises that it will be no secret that he is ill after this is over. He thinks about the promise he made with Raoh and how he will now do anything to keep it.



Soon the last of the missiles fell and not much was left. Civilization and culture were mostly destroyed. The new age began, where violence ruled and those with power were kings. Years later, one place that hadn't been destroyed was the Hokuto Shinken dojo.



And at the dojo are Toki and Sara. Toki sees a flower growing and is surprised that new life could survive here. Sara asks him what he's doing all the way out here as he should be resting. Toki says he is alright, mostly thanks to the treatment she has been giving him. She mentions that Kenshiro has now been chosen to be the successor to Hokuto Shinken and he has accepted it, so Toki must be happy. Toki says he is, but some people think Kenshiro is weak. They don't understand that life isn't just about survival, it's about the meaning of life, to be a human being. His slightly cryptic words give Sara something to think about, but he then announces that it is time for him to go.



Sara can't believe Toki is going on a journey in his condition, but he says that he will die eventually anyway. Later on, he meets Yuria and tells her what he plans to do: he will use his Hokuto Shinken to heal the wounded and the sick for as long as possible before he himself dies. He presents Yuria with the flower he saw earlier and asks her to look after it for him. If she does it will bloom again next year. He asks her to think of him when it does. He then turns around and starts on his journey.



Toki has just set off when Sara asks him to stop. She begs for him to let her come along, as the nuclear war killed everyone in her family and as a doctor, she could help him. Toki warns that this journey is going to be a long and dangerous one, but Sara says she is more than ready. Toki lets her and soon the two come across a destroyed city.



But as the days go on Toki is soon doing what he set out to do: help sick people. But the stuff he warned Sara about is also a problem, such as them having to get through some fierce weather as they travel from village to village...



..and of course Toki is a sick man, so whenever he falls down he's thankful Sara is there to help him back up. As these two are traveling...



Kenshiro is using Hokuto Shinken to protect the weak. It didn't take long for the people he protected to call him their savior. At the same time, Raoh, who had taken the name of Ken-Oh (Fist King), started his plans of conquest. But there was one man he believed who could stop him, and that man was Toki.



Raoh sends his forces to a village Toki is at. His men threaten to kill everyone unless Toki comes with them, so Toki has to leave Sara behind and give himself up. He gets imprisoned at a place known as...



...Cassandra prison. But it doesn't take long for Kenshiro to find out where Toki is. One day two guards come to Toki's cell and tell him that he is being moved out of here as their master, Raoh, doesn't want Kenshiro to meet Toki again. But Toki tells them that they are too late, Kenshiro is already here. And a couple of seconds later, the wall behind them explodes.



Kenshiro steps out. The two guards threaten to kill Toki, but Ken says that they don't understand what kind of man Toki is and that they should try and kill him, if they can. The guards say Ken-Oh told them to execute Toki if it had to come to that, so they swing their swords at him only for him to stop them with his fingers. He then strikes back with a Hokuto Shinken technique.

Toki tells the guards that they will at least die painless deaths. The guards begin to twist and bulge up, but both say they don't feel anything wrong at all, but they then explode. Kenshiro heads over to Toki and says that he looks very ill, but Toki says he is at least alive.



At Ken-Oh's palace, Souga reports to his master that Cassandra has fallen. Kenshiro has released all the prisoners there, and Toki has also been rescued. He asks Raoh why he didn't kill Toki when he had the chance because now, if he sides with Kenshiro, then he is potentially an even greater threat than the Holy Emperor, Souther. He thinks Raoh spared him because he is his brother and has the same blood as him, but Raoh tells him he got rid of such compassion a long time a go. Losing interest in talking about Toki, he tells Souga that their main goal is to defeat Souther. He sends Souga off to try and find out what Souther's secret is. As he leaves the room, Raoh says that the time to fulfil the promise be made with Toki is fast approaching.
